The Consulate General of the Republic of Poland in Chicago is organizing a consular session in Minneapolis on Saturday, September 27, 2025. The visit will take place at John Paul II School at Holy Cross Church, 1630 4th St NE, Minneapolis.

During your visit, you will have the opportunity to submit a passport application. Advance telephone registration is required. Registration will be conducted on September 16 and 17. To schedule an appointment for a passport application, please call the Consulate at 312-337-8166, ext. 1, between 10:00 AM and 1:00 PM Chicago time (or until all slots are filled).

Adres szkoły: 1630 4th St NE, Minneapolis. Podczas dyżuru będzie możliwość złożenia wniosku o wydanie dokumentu paszportowego. Na dyżur obowiązują wcześniejsze zapisy telefoniczne. Zapisy będą prowadzone w dniach 16 i 17 września. W celu dokonania zapisu na wizytę paszportową, należy zadzwonić do Konsulatu pod numer telefonu 312 337 8166, wew. 1, w godz. 10.00-13.00 czasu chicagowskiego (lub do wyczerpania miejsc).
