Polish Consul Visit in Minneapolis

You need to book your appointment ahead of time. This has to be done on October 6, 7, or 11.
Please call Polish Consulate in Chicago at 312 608 9229 between 10am-12pm.
During the duty hours it will be possible to apply for a passport.

Na dyżur obowiązują wcześniejsze zapisy telefoniczne. Zapisy będą prowadzone w dniach: 6 i 7 oraz 11października 2022 roku.
W celu dokonania zapisu na dyżur nalezy dzwonić do Polskiego Consulatu pod numer telefon: 312 608 92 29, w godz. 10.00-12.00.
Podczas dyżuru będzie możliwość złożenia wniosku o wydanie dokumentu paszportowego.
